Consent and Waiver
PLEASE READ THE FOLLOWING CAREFULLY
RELEASE AND WAIVER: The undersigned (“Client”) is the owner of the animal(s) described herein (the “Pet”). The Client hereby authorizes and permits the Vet Tech 2 U and all associated parties and the Company owner(s), technicians, assistants, members, investors, officers, managers, employees, contractors, and agents, a Florida limited liability company (the “Company”), to provide the treatment and/or services for the Pet(s) set forth on the Services Selection Form attached hereto and incorporated herein (the “Services”). Except for claims, actions, costs or expenses arising out of the gross negligence of the Company, its contractors, representatives and agents (the “company indemnified parties”), the Client hereby waives and releases any and all claims and causes of action against the Company indemnified parties and shall indemnify, defend, and hold harmless the company indemnified parties from and against any and all liability, cost and expense caused by or arising out of injuries or damage by the Pet, to itself, humans, other animals, real property, and personal property in connection with the performance of the services. The Client understands, acknowledges, and agrees that he/she is solely responsible for and shall pay any and all costs incurred in connection with emergency services that are provided to the Pet. If the Company pays for any emergency services for the Pet, the Client shall reimburse the Company for any such sum immediately upon demand and presentation of a receipt; provided, however, nothing contained herein obligates the Company to front any costs arising out of any emergency services. The Client further understands and agrees that he/she has an on-going and continuing obligation to update the Company, and the disclosure form contained herein with any subsequent health issues and/or safety related incidents with the Pet.
The Client shall pay all charges for the Services requested pursuant to the payment policies of the Company, as the same may be modified from time to time.

CONSENT: By acknowledging this consent, the Client hereby voluntarily gives permission for their Pet to have services performed by the Company. The Client understands that the treatment(s) provided or any information that is given in no way substitutes for regular/primary/general practice or emergency veterinary care provided by a licensed veterinarian in a clinic or hospital, and that the Client seeking these treatments must be under the approval of a licensed veterinarian. The Client understands that any information given by the Company that is not given by a currently Licensed Doctor of Veterinary Medicine should only be used for educational purposes, and if any concerns arise the Client should take their Pet to a Veterinary Clinic or Hospital for care under the direction of a currently Licensed Veterinarian, and therefore release the Company from any liability for any issues or complications that do arise from any information or procedures provided. The Client understands that the Company is a separate entity from any other veterinary hospital or clinic where any of its associates may also work and should not be affiliated in any way.

ANIMAL UNPREDICTABILITY AND VOLATILITY: The Client understands that every Pet reacts differently and that animals by nature are unpredictable. The Client understands and acknowledges that any animal may bite or cause other injuries to humans and other animals without warning or reason. The Client acknowledges and understands that there are certain risks involved in a social or personal environment while having services and treatments done. The Client understands that if their pet(s) is/are reactive or aggressive or there are other conditions that disallow the services to be performed, the Company reserves the right to refuse service and there is a non-refundable fee for the time taken to attempt the services on the Pet. I release the Company from any liability for any property damage done by the Pet and from any medical or legal liability caused by or to the Pet.

HEALTH RESPONSIBILITY POLICY: The Client understands that all pets must legally have an up-to-date rabies vaccination (for species of animals required to obtain an up-to-date rabies vaccination), and the Client must provide proof of this vaccination every time it is updated. If the Pet has vaccinations waived for a medical reason by a Licensed Veterinary Doctor, the vaccination waiver must be provided. The Company recommends that all pets be on a flea / tick preventative program, and if any fleas, ticks, or parasites are noticed, the Company will notify the Client and will recommend a currently licensed Veterinary visit for treatment. In the event the Clients’ Pet health requirements as stated are not up-to-date or do not have the vaccination certificate, the Company reserves the right to deny services to the Clients' Pet. Should a request be made to provide services for an additional animal, the Company reserves the right to refuse service if the necessary documentation for that animal has not been presented. It is the client's responsibility to ensure all required paperwork is provided prior to the appointment for each animal to be serviced, for the safety of all individuals involved.

ADDITIONAL SERVICES AND CHARGES: Should additional services be requested during the course of the scheduled appointment, and if the Company is able to accommodate such requests, an additional charge will apply. The client will be informed of any extra fees prior to the commencement of these additional services. All charges will be added to the final invoice.

​If the Client does not agree to the above Terms & Conditions, the Company will not be able to provide services. ​
